Early Peripartum predictors of CArdiovascular REverse reMOdelling in hypertensive pregnant woMen

Cardiovascular risk factors have a significant impact on pregnancy-induced (reverse) cardiac remodeling. The most significant cardiovascular changes occur in the immediate postpartum period, when the most severe alterations in LV geometry, diastolic function, and myocardial deformation in women with persistent hypertension after hypertensive disorders of pregnancy (HDP) have already been documented in the literature.

The primary objective of CAREMOM is to recruit healthy pregnant women and hypertensive women in the third trimester to characterize cardiovascular reverse remodeling (RR) during the peripartum period and explore how this may predict LV mass regression 6 months postpartum. Secondarily, CAREMOM will implement a strategy of daily blood pressure monitoring after delivery. This strategy aims to optimize antihypertensive pharmacological therapy in hypertensive women up to 1 month postpartum and assess its impact on cardiovascular RR 6 months postpartum. As a tertiary objective, we aim to clarify the effect of vascular function, parity, offspring sex, breastfeeding, and pregnancy-derived neuroendocrine agents (estrogen, progesterone, relaxin-2, and oxytocin) on cardiovascular RR from early postpartum to 6 months postpartum.
